Important Forms
Pack Reimbursement Form - please fill out and submit this form to our Treasurer for reimbursement of expenses (all expenses should be pre-approved by Pack leadership)
BSA Adult Application - this is the hard copy form used to apply to the Pack and become a leader in the BSA
BSA Adult Application Council Background Authorization form - this is required as part of the Adult application for the Council
BSA Adult Application Youth Protection Training - this is a required online training to work with youth within the BSA (every 2 years)
BSA Adult Application AB506 Mandated Reporter Training and Background Check Authorization and LiveScan - required by the State of California for all adults working with youth for more than 16 hours a month or 32 hours per year (includes driving Scouts beyond your own)
BSA Youth Application - this is the hard copy form used to apply to the Pack and become a Cub Scout in the BSA
BSA Annual Health & Medical Forms - these are required to ensue we keep all Scouts safe during meetings and on outings
